To avoid any processing delays, please be certain to complete the application form in its entirety and include the following with your submission:

1. A copy of the Declarations page from your current Professional Liability Insurance Policy
2. A copy of your letterhead
3. Fax number and/or e-mail address for each of your lawyer references (See Part 7 of application)
